The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Case Manager 3
- Behavioral Health Unit in the Specialty District. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

Job Duties

Essential Duties and Responsibilities include but are not limited to:

  • Completes progress notes, facilitates and writes ISPs and reviews of ISPs and other required reports, letters, and other correspondence as needed. Maintains an organized and up to date case file in accordance with agency policy. Completes support plans, forms, reports to comply with Arizona Long Term Care (ALTCS), Targeted requirements (TSC), and state funded only requirements (DD Only).
  • Conducts field visits per agency policy requirements at various locations: family homes, group home, other residential placements, psychiatric hospitals, schools, day programs, work programs, hospitals, etc. to develop relationships with the child/adult and families.
  • Monitors appropriateness and safety of the individual's placement, assesses mental health status, strengths and needs. Extensive problem solving and conflict resolution within member teams. Works in conjunction with other team members and the supervisor to determine appropriate case plan goals.
  • Attends discharge staffings at psychiatric facilities, petitions and attends hearings for Mental Health Court Ordered Treatment, attends Child Family Teams (CFT) for children in residential group home placement. Works closely with various mental health agencies and with the local psychiatric facilities in managing member services and in assisting in ensuring a prompt discharge when member has been involved in an in-patient stay.
  • Compiles information from a variety of sources in order to make recommendations for re‑determinations. Evaluates and assesses the need for higher behavioral health rates, enhanced staffing ratios for day programs, referrals for positive behavioral supports for members.
  • Monitors progress of services through review of written reports, meetings and telephone contact. ALTCS review dates, completes cost effective studies (CES). Updates demographic information. Authors reports.
  • Works with DCS case managers with children in residential group home settings. Be knowledgeable about all colleagues' members. Step in and attend meetings for peers, confer with Supervisor, may be called upon to act as supervisor in supervisor's absence.
