Key Responsibilities:

• Provide primary health care services in diverse settings

• Deliver virtual care and home visits when necessary

• Collaborate with the Family Care Team for outreach support

• Maintain effective relationships within the interdisciplinary team

• Travel to different medical clinics as required

Requirements:

• Registered Nurse and Nurse Practitioner Licenses required

• Strong communication and interpersonal skills

• Ability to work independently with minimal supervision

• Willingness to travel to assigned locations

• Proven record of work performance and attendance
