Summary:

The University Of North Carolina Hospital Department Of Pharmacy has an immediate opening for a highly motivated individual interested in providing patient care to our pediatric patient population. This pharmacist will work in a multidisciplinary environment to provide comprehensive pharmaceutical care to multiple services including Hematology/Oncology, Bone Marrow Transplant, Intensive Care (Pediatric, Cardiac, and Neonatal), Pulmonary/Cystic Fibrosis, Endocrine and Cardiology. This position is primarily day shift (7am-3:30pm), but will be required to work some evening shifts based on the needs of the team.

The position will be responsible for precepting students from the UNC Eshelman School of Pharmacy as well as both PGY1 and PGY2 residents.

UNC Children s clinical home, N.C. Children s Hospital, is a major referral center for children with complex conditions and consistently recognized by U.S. News & World Report as one of "America s Best Children s Hospitals" on its annual list. N.C. Children’s Hospital has 172 inpatient beds and a comprehensive children s outpatient center located in the same building, enabling patients to see multiple specialists in a single visit.

The Pediatric Services at UNC Hospitals and Clinics are in a period of growth. Our mission for the pediatric pharmacy is to provide quality, patient-centered care, in collaboration with other healthcare professionals, for all pediatric patients within the Children’s Hospital by practicing evidenced-based medicine, optimizing medication use and distribution while ensuring medication safety.

The Department of Pharmacy is a dynamic and critical part of patient care provided by the UNC Health Care system. UNC’s Department of Pharmacy is an organization of more than 500 dedicated employees with focused attention on caring for patients of the UNC Health Care system. The Department is highly interactive with the University of North Carolina Eshelman School of Pharmacy, located on the same Chapel Hill campus.

UNC Hospitals and Clinics acts as a major clinical and administrative training site for these pharmacy students, in addition to its greater than 40 pharmacy residents.

Qualified candidates will have earned a BS in Pharmacy or a Doctor of Pharmacy degree from an ACPE-accredited school/college of pharmacy and must be licensed or eligible for licensure to practice pharmacy in the State of North Carolina. Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience is also preferred. Candidates should possess and demonstrate excellent initiative, communication, analytical skills, organization, and collaboration.

Responsibilities
  • Ensures timely medication access for service patients; including order review and verification. Clarifies incomplete, inaccurate, or contraindicated prescriptions with the prescriber, nurse, or other health professional, as appropriate. Identifies and intervenes on problems related to the medication needs of the patient.
  • Utilizes and analyzes patient data to provide optimal drug therapy recommendations. Assesses the patient s medication therapy regimen and makes appropriate clinical interventions with the provider.
  • Performs protocol-driven clinical services such as therapeutic dose monitoring, antimicrobial stewardship activities, etc.
  • Compounds and oversees the preparation and dispensing of medications including sterile products using standard formulas, processes and procedures as required.
  • Maintains patient profiles within electronic medical record to facilitate communication
  • Complies with the medication formulary, and therapeutic substitution and other programs designed to optimize drug therapy while minimizing cost as required.
  • Facilitate discharge and transfer of patients (ie. medication reconciliation and discharge counseling).
