American Traveler is hiring an experienced RN Case Manager for an acute care hospital setting, requiring NC or compact RN licensure and prior care/case management experience.

Requirements
  • Active RN license in North Carolina (Standard or Compact)
  • 2+ years of Care Management or Case Management experience in an acute care setting required
  • 2+ years of healthcare experience as a Registered Nurse
  • Nursing Diploma, ADN, or ASN from an accredited nursing program required; BSN or MSN preferred
  • Strong assessment and critical thinking skills required
  • Experience with Epic EMR preferred
Responsibilities
  • Acute care hospital Care Management department covering a 20-25 bed unit
  • Patient population includes Cardiology, Nephrology, Trauma Surgery, Orthopedic Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
  • Role may expand to include Women & Children's services
  • Uses Epic EMR for documentation and care planning
  • Day shift schedule, Monday through Friday, 8:30 AM to 5:00 PM
  • No weekends, holidays, or on-call requirements
  • Coordinate comprehensive care for acutely ill patients with complex medical conditions and discharge planning needs
  • Conduct patient assessments and document individualized care plans in Epic
  • Participate in daily Care Management touchpoints and CAPP meetings to monitor patient progression and discharge readiness
  • Attend weekly Complex Care Meetings to collaborate on high-risk and complex patient cases
  • Coordinate with Social Workers, Utilization Managers, and Care Management Assistants on patient care and discharge planning
  • Educate patients and families on community resources and coordinate referrals to post-acute providers
  • Experience in an academic or teaching hospital environment is helpful
