About the Role We have an exciting opportunity for one part time Short Breaks Personalisation Officer to join our busy team! The Short Breaks Personalisation Officers play a vital role in supporting families and disabled children/young people to access person centred, appropriate short breaks provision.

The Person alisation Officer’s responsibilities can be split into two main areas:
1.Enabling access to Short Breaks Services for disabled children and young people in line with pathway one and two provision:
Processing applications made directly by families/professionals and de-escalations from the Children's Disability Service Screening for eligibility criteria Assessing families needs through utilising application forms, professional supporting letters, liaison with schools and other agencies and the Resource Allocation System (RAS). This is including the completion of home visits, and other visits necessary to gain a holistic view of the child/young person and capture their voice Identifying level of support needed and indicative budget Completing annual reviews for Pathway Two support packages and processing Pathway One re-applications, to ensure services are still appropriate and meeting needs Following up and answering short breaks enquiries
2. Assisting and enabling service users to use their Personal Budget to meet their social care and or health care needs, by helping them plan and source both paid support, and unpaid informal support from their social networks and communities. Completing person centred support plans and other mandatory documentation to request and refer to the appropriate identified services in line with the child/young persons identified SEND outcomes.

About You The ideal candidate will have high standards of communication with vulnerable adults and children, as well as being able to work in partnership with other professionals and agencies and the ability to communicate in varied ways with disabled children and young people.

The Person alisation Officer is expected to work directly with citizens to identify appropriate support and provision, in line with their personal support plan / care plan and direct payment (if relevant). Please ensure that the personal statement in your CV demonstrates the following: ESSENTIAL REQUIREMENTS:
Knowledge of Personalisation and Person Centred ways of working Knowledge of the SEND outcomes framework Ability to work in a person-centred way to develop individual care and support plans, which meet identified outcomes (inclusive of conducting home visits)

Experience of managing and prioritising own workload, working independently and recognising when support from your line manager is required Experience in maintaining electronic case files and administrative systems A DBS enhanced check for a regulated activity is required for this post.
