MDS Care Plan Nurse Opportunity LPN/RN
Hilltop Skilled Nursing and Rehab Center is looking for a MDS Care Plan Nurse RN/LPN to join our growing team!
The MDS Coordinator is a key member of the Hilltop Leadership Team who will integrate and coordinate a patient‑centered nursing strategy focused on quality patient care. The MDS Coordinator’s primary focus is the resident assessment process, which includes individual assessment, MDS assessment, and resident plan of care. The MDS Coordinator manages the resources provided to residents and coordinates the entire continuum of care.
Responsibilities- Provide accurate documentation of all services to assure accurate reimbursement from all payor sources.
- Ensure that needed resources are available and that quality care is delivered to all residents.
- Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.
- Must have a current RN or LPN license.
- Must have long‑term care experience.
- 1–2 years of MDS experience preferred.
- Must be knowledgeable of long‑term care nursing and medical practices and procedures.
