Position: Initial Service Coordinator

205 Days per School Year

SUMMARY

Under the direct supervision of the Director, Early On and Related Services, and in accordance with established policies and procedures of Eaton RESA, the Initial Service Coordinator shall provide child (birth to age
3) and family assessment, identify direct and indirect instructional services, family-centered services, and collaborative consultation with staff, families, agency personnel, community providers and administrators. This position will interface with local school district and ERESA staff, community child care/preschool providers, agency personnel and medical providers.

PRINCIPAL DUTIES AND RESPONSIBILITIES
  • Assures that early intervention evaluations are provided according to state and federal laws related to IDEA Part C and Michigan Mandatory Special Education (MMSE).
  • Upon receiving an initial referral, completes an intake interview and coordinates initial evaluations with appropriate evaluators, sharing documents with the evaluators.
  • Evaluates children for Part C, MMSE, and Part B eligibility as appropriate.
  • Ensures all documentation is complete for the initial eligibility including eligibility determination, initial IFSP, and Entry Child Outcome Ratings.
  • Serves as the family’s initial Service Coordinator throughout the referral and initial evaluation process.
  • Participates in team meetings, sharing information on referrals and engaging in team discussion to determine assignment of a Primary Service Provider.
  • May schedule Part B evaluation for children referred to Early On close to age three, when Early On evaluation is not appropriate.
  • Works with Director and Administrative Assistant to ensure that data is maintained on all referrals in order to maintain Part C and Michigan Mandatory Special Education Compliance.
  • Maintains good relationships and open communication with team members, administration, and community stakeholders including local school districts, agency personnel, medical providers, community providers, young children and families.
  • Prepares and presents in-service programs for team members, parents and community stakeholders.
  • Prepares, maintains and processes required program documents, records and reports.
  • Maintains up-to-date knowledge of research, theories and practices associated with early intervention.
  • Other duties as assigned.

QUALIFICATIONS KNOWLEDGE, SKILLS & ABILITIES
  • Knowledge of special education and Early On rules, laws, and regulations, and ability to provide technical assistance to other staff.
  • Knowledge of all domains of child development to assess needs and develop interventions. Ability to participate in Part C, Michigan Mandatory Special Education (MMSE), and Part B evaluations as a member of the multidisciplinary team.
  • Knowledge of family-centered services and practices to support and address family resources, needs, concerns and priorities, and ability to provide technical assistance to other staff.
  • Ability to work and communicate effectively with administrators, staff, agencies, medical providers, community providers, young children and families in the role of service coordinator.
  • Knowledge of family-centered services and practices to support and address family resources, needs, concerns and priorities, and ability to provide technical assistance to other staff.
  • Organizational ability, self-directedness, and initiative when given a variety of responsibilities and assigned tasks.
  • Ability to lead, coordinate and make decisions and problem solve independently and collaboratively within a team of service providers in partnerships with the family.
CERTIFICATES, LICENSES, REGISTRATIONS (If Applicable):

Required

  • Possession of one of the following:
    Michigan Teaching Certificate in special education or early childhood, or Michigan Licensure in one of the following fields:
    School Psychology, Nursing, School Social Work, Speech‑Language Pathology, Occupational Therapy, or Physical Therapy.
