Position Title: Street Outreach Coordinator

Reports to: Economic Mobility Manager

Classification: Nonexempt

Category: 40 hours per week; nights and weekends regularly necessary.

Location: Primarily community based with some in-office work required.

Job Summary

Under the general direction of the Economic Mobility Manager and in accordance with the mission of the Ada Jenkins Center, while respecting and honoring the values and ethics set forth by the social work profession, the Street Outreach Coordinator will provide comprehensive assessment, outreach, diversion, and referral for individuals and families experiencing homelessness. The Street Outreach Coordinator will identify unsheltered individuals and families residing in a place not fit for human habitation such as on the street, cars, tents, etc.

in Mecklenburg County with a focus on the Davidson, Cornelius, and Huntersville area. To be successful in this position, the individual must be able to manage time with multiple competing demands and work collaboratively on issues in a positive, solution-focused manner. In addition, a high level of confidentiality must be maintained at all times.

Job Qualifications
  • Bachelor's degree from an accredited college or university of Social Work, or related field, preferred. High School diploma or GED, required.
  • Must have demonstrated knowledge of outreach methods, interviewing and assessment techniques, and referrals.
  • Working with the homeless community: 2 years required
  • Demonstrated knowledge and commitment to advocate for client's needs.
  • A valid driver's license and reliable insured transportation are required.
Skills, Abilities and Attributes
  • Strong organizational skills and the ability to set priorities.
  • Creative problem‑solving skills.
  • Ability to work as part of a team, as well as independently.
  • Strong interpersonal skills, cultural competence and ability to employ sensitivity in building rapport with clients.
  • Strong written and verbal communication skills.
  • Computer proficient in Microsoft Word, Outlook, and Excel.
  • Experience with data tracking systems is helpful.
  • Prior working with volunteers.
  • Must have a passion and genuine commitment for the mission of the Ada Jenkins Center.
PRINCIPLE FUNCTIONS - STREET OUTREACH COORDINATOR Street Outreach Services
  • This position will be working in Mecklenburg County with a focus on the Davidson, Huntersville, and Cornelius area.
  • Locate, identify, and visit clients in the woods, places not meant for habitation, and outside.
  • Use client-centered approach to build relationships with those experiencing homelessness to include assessment and referrals to community-based resources and housing.
  • Assist clients with procuring necessary documents and services such as identification card, birth certificate, social security income, disability income.
  • Identify appropriate emergency/transitional /permanent housing options for clients such as emergency shelter, subsidized housing, HCV (Housing Choice Voucher, VASH and other housing opportunities.
  • Assist with housing applications/paperwork.
  • Complete Coordinated Entry for unsheltered individuals.
  • Develop partnerships with other agencies to advocate for clients' housing and other needs
  • Enter information into the Homeless Management Information Services database within 48 hours.
  • Attend collaborative meetings, as requested
  • Actively participate in team meetings and trainings.
  • Other duties as assigned.
Legal Aspects
  • Maintain confidentiality of all program documents and records.
  • Adhere to NASW Code of Ethics.
  • Maintain participant documentation, including all financial and programmatic records, for a period of at least five years in accordance with 24 CFR 578.103 (c).
Administration
  • Work hours vary and will be set based on clients' needs. Some nights and weekend work will be necessary.
  • This position will work primarily in the community, but in-office days are required.
  • Attend and actively participate in interagency staff and multidisciplinary meetings.
  • Compile and submit reports on a monthly basis, or as assigned, to Economic Mobility Manager.
