Purpose and Scope

The professional registered nurse Outpatient RN CAP 1 is an entry-level designation into the Clinical Advancement Program (CAP). This position is responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegating appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations.

Principal

Duties and Responsibilities
  • Perform all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses.
  • Conduct ongoing, systematic collection and analysis of patient data before, during, and after hemodialysis treatments, document in the medical record, and make necessary adjustments or notify relevant team members.
  • Assess and document patient/family’s learning needs to provide initial and ongoing education.
  • Coordinate and deliver safe, effective hemodialysis therapy in collaboration with care staff.
  • Administer medications as prescribed or per approved protocols, documenting medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource for the healthcare team, participate in staff training and orientation.
  • Ensure proper laboratory procedures are followed, including collection, processing, and shipping, and reschedule missed collections.
  • Update nursing assessments and care plans through collaboration with the interdisciplinary team.
  • Provide patient education regarding transplant and treatment options.
  • Complete CAP requirements for career advancement.
  • Perform other duties as assigned by the Supervisor.
Physical Demands and Working Conditions

The physical demands and environment are typical of a healthcare setting. Reasonable accommodations will be provided for individuals with disabilities.

  • Heavy lifting, moving patients, and assisting with ambulation, with equipment or coworker assistance as needed.
  • Prolonged standing and bending required.
  • Occasional movement of equipment up to 200 lbs., lifting chemical and water solutions up to 30 lbs. to a height of 5 feet.
  • Work environment is a healthcare facility with controlled temperature and moderate noise.
  • Potential exposure to infectious materials.
  • Includes desk work, computer tasks, and interaction with patients and staff.
  • Travel may be required for training or other facilities.
  • Essential functions may need to be performed in different locations with similar physical demands.
  • Employees must pass the Ishihara Color Blindness Test; accommodations will be considered if needed.
Supervision

Oversee Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or Nurse in Charge after meeting all training and experience requirements:

  • Complete all Fresenius Kidney Care training requirements.
  • Minimum of 9 months RN experience.
  • Minimum of 3 months in chronic/acute hemodialysis as an RN.
Education and Licensure
  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS certification.
  • Meet state practice requirements.
Experience and Skills
  • Entry level for RNs with less than 1 year of recent Nephrology Nursing experience.
  • Minimum of 9 months RN experience preferred.
  • Chronic/acute hemodialysis experience preferred.
