- Provide comprehensive medical care to HPCCR patients and serve as a medical resource for HPCCR staff.
- Promote the highest quality of end-of-life care including hospice care and the use of advanced directives.
- Diagnose, treat, and manage pain and other symptoms experienced by patients at the end of life.
- Guide and counsel patients and their families regarding disease progression, prognosis and the benefits/burdens of therapeutic measures, tests, procedures, and drugs.
- Prescribe and/or recommend therapeutic measures, tests, procedures, and drugs in accordance with ethical principles of medicine.
- Plan for situations beyond the nurse practitioner’s expertise and coordinate with the Medical Director or Medical Services Physician as appropriate.
- Serve as an end-of-life care education resource within the organization and the community.
- Pursue educational opportunities to improve patient outcomes.
- Evaluate patients through history and physical examinations; discuss findings with attending physicians and healthcare team.
- Prescribe systemic and topical drug therapies including narcotics for pain and other symptoms.
- Conduct patient/family meetings to review medical issues and establish goals of care.
- Provide psychosocial support and refer for diet/exercise therapies; recommend non-pharmacological therapies (e.g., massage, physical therapy, occupational therapy, music therapy).
- Recommend appropriate diagnostic tests and studies; review advance care directives; refer to hospice services when appropriate.
- Participate in team discussions to gain insights and suggestions; stay up-to-date on current trends in hospice and palliative medicine.
- Submit billing and documentation according to policy and assist in policy development to optimize care and reimbursement.
- A Master Degree in nursing; current, active, and unrestricted NC Registered Nurse (RN) license and official recognition as Nurse Practitioner from NC Board of Nursing.
- Successful board certification in specialty within 2 years of date of hire; current and unrestricted DEA and Basic Life Support required; computer proficiency.
- Minimum 1 year related experience. Experience in direct care of patients with life-limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics; oncology; home health care; medical-surgical.
