Title: Family Nurse Practitioner (FNP) or Physician Assistant (PA)
Location: Charlotte, NC
Position Type: Contract-to-Hire
Schedule: 8-Hour and 10-Hour Day Shift Mix
Compensation: $70.00-$75.00 Per Hour
- 1 year of recent FNP experience in Community Health/Federally Qualified Health Center/Primary Care/Family Health/Urgent Care is required.
- Medicaid Enrollment Required
- Recent experience working with underserved communities.
- Board-certified FNP with active North Carolina license.
- Proficient in EMR documentation and comfortable seeing patients of all ages.
- Strong communicator with a patient-first mindset.
- Must have or be eligible for a DEA license.
- CPR certification.
- We're working with a mission-driven community health center in Charlotte, NC, that's looking to add a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join their compassionate care team.
- This role offers the chance to make a real impact by delivering comprehensive, primary care services to patients from birth through adulthood with a focus on prevention, chronic disease management, and continuity of care.
- Provide primary care to 16 patients a day in a team-based model.
- Diagnose, treat, and manage acute and chronic health conditions across the lifespan.
- Order labs, imaging, write prescriptions, and make referrals.
- Collaborate closely with physicians and integrated care teams (RNs, MAs, Behavioral Health, etc.).
- Document in EMR, follow clinical guidelines and participate in QI initiatives.
- Support community health efforts and connect patients to local resources.
- Attend provider meetings and participate in clinical evaluations/peer review.
- Finish early on Fridays!
- Serve underserved communities and help close gaps in care.
- Collaborate in a supportive, team-based care model.
