Registered Nurse - Home Health West Charlotte

Job Overview

Registered Nurse (RN) – Home Health – Charlotte, NC – Full‑time (40 hrs/wk). This role provides direct patient care and supervision of LPNs and HHAs in the home setting, ensuring comprehensive case management and coordination with the multidisciplinary team.

• Manage daily caseload of home health patients, ensuring timely care and accurate documentation per plan of care and MD orders.
• Collect and analyze patient data through home assessments, telephonic contacts, and EMR information to identify high‑risk patients and facilitate effective resolution.
• Supervise LPNs and HHAs, directing coordinated care and promoting safe outcomes.
• Perform ordered treatments, therapies, and procedures in accordance with state nurse practice act and home health conditions of participation.
• Maintain current RN license and BLS certification; BSN preferred, and complete any additional education or training as required.

Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 lbs. Physical Abilities Testing required. Must speak English in good, understandable terms.

Graduate from an accredited School of Nursing. Current RN license required; BSN preferred. BLS required.

Competitive salary range $34.90–$52.35 per hour. Sign‑on bonus up to $15,000 and referral bonus $7,500. Charlotte – Metro/West Charlotte area. Monday through Friday, with some call evenings and weekend coverage.
