Advocate Health is seeking a Home Health RN to manage a patient caseload in the University City area. You will coordinate care across a multidisciplinary team and ensure timely documentation and adherence to the plan of care.
The role requires an RN license with BSN preferred, and BLS certification. You will work Monday–Friday with on-call rotation and cover Mooresville, Cornelius, and Davidson areas in North Carolina.
