• Assist with coordination and implementation of individual support plans for adults with developmental disabilities.
• Support full community access and participation based on personal choices and interests.
• Provide support and skill-building services as per each individual’s ISP and program schedules.
• Attend team and professional meetings, representing the program professionally in the community.

Reports to Program Manager or Director. Works independently within standard practices, exercising sound judgment, initiative, and leadership. Collaborates with Direct Support Professionals to ensure compliance with state standards, Medicaid Waiver regulations, and organizational policies. May be assigned to various locations as needed.

• Valid Virginia Driver’s License with acceptable driving record.
• Personal automobile insurance; own car available daily, in safe condition, with capacity for three passengers.
• Four-year degree in human services preferred; minimum of high school diploma or GED.
• Ability to respond to emergencies, perform physical tasks including lifting, transferring, personal care, and approved restraints, and navigate stairs and community environments.

Meadow creek Center, 2000 Michie Dr., Charlottesville, VA 22901.

Grade: 8.

Schedule:

40 hours/week, Monday–Friday, shifts may vary including evenings and weekends, most days 8:30 am–4:30 pm.
