Support Coordinator Region I REACH CTH (FULL TIME)

General Statement Of Responsibilities

The incumbent serves as a Region 1 Regional Education Assessment Crisis Services Habilitation (REACH) CTH Case Manager for adults and children with developmental disabilities and behavioral health needs in addition to those presenting with challenging behaviors (DD/BH) residing in Developmental Services Region I of Virginia which covers the nine Community Services Boards (CSBs) within Region
1. This position provides support to persons aged 18 and over with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing crisis or planned therapeutic stays within the REACH Crisis Therapeutic Home. This position also provides support to children and adolescents with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing crisis or planned therapeutic stays within the Youth REACH Crisis Therapeutic Homes outside of Region I.

This position participates in the daily routine of the home and provides support for therapeutic intervention, habilitative activities, personal care, health care and other activities.

Qualifications

Qualified Mental Health Professional (QMHP or QMHP-T)– minimum of Bachelor’s degree in human services or related field from an accredited institution and no less than one year of full-time relevant experience. Master’s Degree in a human service field (counseling, special education, psychology, social work, or closely related field) preferred. Valid Virginia Driver's License plus an acceptable driving record as issued by the Division of Motor Vehicles is required.

The incumbent must be able to perform all essential duties.
