The licensed social worker administers social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care.

• Master’s degree from an accredited school of social work.
• Certified or licensed in the state of practice.
• Minimum one year of social work experience in a health care setting.

• Medicare home health or hospice experience.
• Volunteer coordination experience.

Alabama, Alaska: BSW may work under an MSW; MSW preferred.

Arizona:
Licensed BSW under a licensed MSW; LMSW preferred.

Arkansas:
Minimum LMSW required.

Colorado:
Masters‑level LSW required.

Connecticut: MSW required.

Florida:
No licence required, certification only. Provide masters degree transcript.

Georgia:
Minimum LMSW required.

Idaho: BSW under an MSW; MSW preferred.

Illinois: BSW under an MSW; MSW preferred.

Indiana: BSW under an MSW; MSW preferred.

Kansas: LBSW under an LMSW; LMSW preferred.

Kentucky: LBSW under an LMSW; LMSW preferred.

Louisiana: LMSW required.

Maryland: LBSW under a licensed clinical social worker; masters‑level LCSW preferred.

Massachusetts: LSW under an LCSW; LCSW preferred.

Mississippi: BSW under an MSW; MSW preferred.

Missouri:
State licensure not required. BSW under an MSW; MSW preferred.

Montana: BSW under an MSW; MSW preferred.

Nevada: CSW under a CMSW; CMSW preferred.

Ohio: BSW under an MSW; MSW preferred.

Oklahoma:
Masters degree required. Must be an LSW or LMSW.

Oregon:
Registered BSW under an LMSW; LMSW preferred.

Pennsylvania:
Masters degree required. Must be an LSW.

Rhode Island: LBSW under an LMSW; LMSW preferred.

South Carolina:
Minimum LMSW required.

Tennessee: MSW required.

Utah:
Social service worker under a CSW; CSW preferred.

Virginia: LSW under an LCSW; LCSW preferred.

Washington: BSW under an MSW; MSW preferred.

Wyoming: CSW under a CMSW; CMSW preferred.

• Organized and detail‑oriented.
• Effective oral and written communication.
• Capable of making independent decisions relevant to patient needs.
• Prioritizes and handles multiple critical tasks simultaneously.
• Functions well under stress and interruption.

• Valid state driver license.
• Maintain automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safe driving in all weather conditions.
• CPR certification for the healthcare provider.
