As a full- or part-time Physician Assistant - Nurse Practitioner, you will meet with patients to assess their health, develop individualized care plans, and manage medication reduction strategies. A key part of your responsibilities will involve having important goals of care conversations with patients to create long-term health strategies tailored to their needs. While you will work closely with our medical team, you will also have the autonomy to make independent decisions that directly impact the quality of care for your patients.
