Job JOB OVERVIEWThe Designated Nursing Assistant / Certified Nursing Assistant (DNA/C.N.A) is responsible for providing direct resident care including nursing/clinical care, personal, nutritional and restorative care in the skilled nursing operations and other areas throughout the community as appropriate. Responsible for demonstrating the Mission for Sunrise Senior Living, "to champion quality of life for all seniors" in accordance with federal, state and local standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our guests/residents.
RESPONSIBILITIES& QUALIFICATIONS
This role is based in our Skilled Nursing Facility and requires an active Certified Nursing Assistant (CNA) certification through the state of Virginia.
Essential DutiesAs a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. Our foundational belief is the sacred value of human life. The unique responsibilities for this role include but are not limited to the essential functions listed as follows:
Resident Care Duties- Deliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP).
- Report all incidents immediately to appropriate supervisor / team member.
- Report suspected abuse immediately per regulations and Sunrise standards.
- Participate in the care planning for their designated guests/residents.
- Perform individualized personal care and activities of daily living (ADL); documents ADL in flow sheets.
- Partner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests/residents.
- Obtain, document and report the guest/resident's vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested.
- Observe and report any physical and/or psychosocial changes of condition requiring a licensed nurse intervention and care services.
- Assist in maintaining assistive devices, adaptive equipment and other medical equipment to include cleaning equipment and making readily available.
- Maintain Restorative Records for each guests/residents.
- Maintains knowledge of federal, state/provincial and local resident care regulations.
- Strive for excellent quality care and service delivery as measured in the Quality Assurance and Performance Improvement (QAPI) process.
- Competent of behavior interventions and management.
- Demonstrate and knowledgeable of the following key quality improvement areas of the DNA/C.N.A. role:
- ADL, skin care, dining assistance, bowel/bladder elimination.
- Falls and fall prevention.
- Skilled nursing transformation and Resident Centered Care model.
- Respond to guest/resident, family, visitor's inquiries, concerns in a timely and professional manner using the Problem Resolution process.
- Invite, Encourage and Assist (IEA) guests/residents to meals.
- Assist guests/residents when transferring or discharged by collecting belongings, escorting to transportation, other as assigned.
- Greet new guests/residents and escort to their room; orients guests/residents to room/call lights and community layout and provide activity calendar.
