Advanced Practice Provider; NP​/PA​/APP) Ambulatory Cardiology Women's Heart​/General Cardiology- Fontaine

The Advanced Practice Provider (APP) will provide comprehensive clinical care for both Women's Program and General Cardiology populations. In the UVA Women's Heart Program the APP will include Women's Prevention, Cardio-OB clinics, as well as general cardiology services such as HTN, HLD, CAD. The APP will deliver evidence-based cardiovascular care tailored to the patient's needs, serve as a patient advocate, and function as a key member of a multidisciplinary team committed to excellence in patient outcomes, education, and compassionate care.

Shifts:

Monday-Friday, primarily daytime hours; no weekends, holidays or call expected.

• Full-time, outpatient clinical role based primarily in UVA's Ambulatory Cardiology and Women's Heart clinics.
• Day shifts, Monday through Friday.
• Option for four 10-hour days.

• Prior clinical experience in cardiology, internal medicine, or cardiovascular subspecialties.
• Experience with outpatient management of cardiovascular conditions.

• Partner with pharmacists to optimize medication therapy and patient adherence.
• Work with social workers to address behavioral health and social needs.
• Collaborate with registered nurses and navigators for care coordination and follow-up.
• Collaborate with OB/GYN and Maternal-Fetal Medicine for cardio-obstetric care.
• Engage with general and inflammatory cardiology colleagues for shared patient care.
• Collaborate with vascular medicine, endocrinology, and lipidology for integrated cardiovascular prevention.
