Lakeshore Talent is in search of a Home Care LPN for a direct hire opportunity in Charlottesville, VA.

This is an excellent chance to join a mission-driven healthcare team providing essential in-home care services to aging adults. We are looking for a compassionate and dependable Licensed Practical Nurse ready to make a meaningful impact in the lives of patients.

• Schedule: Monday–Friday, 8:00 AM–5:00 PM (with weekend rotation)
• Compensation: $28.85–$36.06/hr
• Must Have: Reliable transportation

Key Responsibilities

• Deliver general nursing care and perform assigned procedures according to company and regulatory standards.
• Administer prescribed medications, treatments, and preventive procedures.
• Monitor and document participants’ health status and responses to treatments.
• Provide participant and family education regarding care plans and medications.
• Carry out orders from primary care and nursing staff.
• Perform phlebotomy and administer vaccines and TB tests when needed.
• Maintain accurate clinical documentation and contribute to care plan updates.
• Participate in peer reviews, quality management, and utilization reviews.

Required Qualifications

• Current LPN license (State Issued)
• First Aid and BLS certification (American Heart Association or Red Cross)
• Valid driver’s license, personal transportation, and auto insurance
• High School Diploma or GED

Preferred Qualifications

• 3+ years of LPN experience in a clinical or long-term care setting
• Familiarity with medications and care specific to the geriatric population
