You will collaborate with an RN, build relationships with patients and families, and ensure comfort, dignity, and quality end‑of‑life care.
You will document care in the EMR, support physicians and RNs, and participate in interdisciplinary teamwork. Frequent travel may be required as part of rounds and community visits.
