Key Responsibilities:

• Conduct client assessments and medication histories

• Provide group and individual education to clients

• Design case management plans collaboratively with clients

• Communicate with other health services as required

• Focus on harm reduction strategies in care delivery

Requirements:

• Baccalaureate Degree or Diploma in Nursing

• Minimum of 1-3 years' nursing experience

• Registered or eligible for registration in Prince Edward Island

• Knowledge of substance use and mental health issues

• Strong interpersonal and problem-solving abilities
