In this casual position, you will be integral to managing client health through detailed assessments and supportive education. You'll work closely with clients to develop personalized care plans while focusing on recovery and family engagement. Interdisciplinary communication is vital to success in this position.
Key Responsibilities:
• Conduct client assessments and medication histories
• Provide group and individual education to clients
• Design case management plans collaboratively with clients
• Communicate with other health services as required
• Focus on harm reduction strategies in care delivery
Requirements:
• Baccalaureate Degree or Diploma in Nursing
• Minimum of 1-3 years' nursing experience
• Registered or eligible for registration in Prince Edward Island
• Knowledge of substance use and mental health issues
• Strong interpersonal and problem-solving abilities
