Key Responsibilities:
• Deliver OT interventions and plan equipment needs
• Participate in interdisciplinary care team meetings
• Facilitate health coaching for clients’ self-management
• Employ digital platforms for documentation and care delivery
• Evaluate home environments for modifications
Requirements:
• Registration with the College of Occupational Therapists of Ontario
• Valid driver’s license for community travel
• Passion for client-centered care and empathy
• Effective critical thinking in assessing client needs
• Advocacy for enabling client independence at home
