Key Responsibilities:

• Travel between sites within the Erie St. Clair region
• Engage with patients, caregivers, and community services
Requirements:
• Current Nurse Practitioner registration in Ontario
• Knowledge of community healthcare resources
• Specialized training in palliative symptom management
• Valid Ontario Driver's License and vehicle access
• Compliance with COVID-19 vaccination policy
