Full-Time Nurse Practitioner in Palliative Care

We are looking for an experienced Nurse Practitioner to provide effective services for community patients and those with limited access to healthcare.

Your role will involve diagnosing, treating, and managing complex issues throughout the palliative care process. You’ll also travel between Erie St. Clair Region sites, collaborating with Hospice to ensure comprehensive support for symptom management.

Key Responsibilities:

• Deliver Nurse Practitioner services to Palliative Care Consultation Team patients

• Diagnose and manage complex pain and chronic diseases

• Ensure adherence to professional standards and Ministry guidelines

• Collaborate with healthcare professionals to improve care outcomes

• Conduct assessments to support care recommendations

Requirements:

• Registered Nurse with current Nurse Practitioner registration

• Experience with community healthcare resources

• Knowledge of palliative pain management practices

• Valid Ontario Driver’s License and access to a vehicle

• Must meet COVID-19 vaccination policy requirements
