Community Care Worker

As a Community Care Worker, you will be responsible for providing care and support to Longevity Care Limited’s service-users in their own homes in line with their agreed care and support plans. This involves looking after their physical, emotional, cultural and social needs using a person-centered approach, observing and promoting the service-user’s choice, independence, dignity and wellbeing privacy, social and cultural backgrounds, fulfilment and other rights.

Community Care Workers also have a duty to maintain good professional relationships with service users and work in partnership with their families, friends and other professionals who are involved in their support and care. Community Care Workers actively support other care workers, bank staff ensuring that they work with empathy, honesty and integrity working in line with all regulatory and statutory obligations and workplace policies, procedures and guidelines

• Providing high quality personal care and support to every service user (in their own homes) taking into account their individual wide range of needs, capabilities, illnesses and disabilities;
• Knowing and understanding the care and support of the service user;
• Undertaking the tasks detailed in the service user’s care and support plan using a person-centered approach and in the least intrusive way;
• Encouraging the independence and motivation of the service user and not fostering dependent behaviour;
• Providing input into the care and support plans of the service user by regularly feeding back to the Field Care Supervisor;
• Assisting service users with their morning routines (e.g. getting up in the morning) and also with night time routines (assisting them when going to bed);
• Assisting service users with their personal care including helping them to dress and undress, wash/bath/shower, toileting needs and continence management:
• Assisting service users to maintain personal hygiene and cleanliness at all times (including looking after their skin, teeth, hair and nails);
• Assisting service users with their medication at the agreed level of support and as detailed in their Medication Care Needs Assessment (specified in their care plans), actively monitoring medication and notifying management of any issues;
• Assisting service users in overcoming any mobility issues or other disabilities including the use of any aids and personal equipment that may assist them
• Assisting with the preparation of food and drink for the service user, cooking of meals, helping with eating and drinking if they have difficulties doing this themselves (being mindful of the service-user’s choices/ preferences, likes/dislikes, nutritional needs and cultural/personal dietary requirements);
• Providing light general household domestic duties, including housework and laundry, as detailed in the care plan or instructed by Management;
• Promoting service-users mental and physical wellbeing including prompting as and when necessary, actively helping with tasks they cannot do themselves and assisting them with any activities they like to do e.greading/writing/watching tv/nail painting, overall helping them maintain their independence.
• Using manual handling equipment safely and correctly;
• Taking responsibility for the safe handling of property and equipment belonging to the service user;
• Maintaining good communication and develop effective working relationships with service users;
• Providing companionship to the service user, actively talking and listening to them about their interests;
• Assisting the service user with maintaining contact with their family and friends;
• Accompanying the service user on trips into the community;
• Assisting the service user to manage their personal affairs;
