Diabetes Educator - Endocrinology - Full-Time
Certified Diabetes Educator or certification eligible RN, RD, Pharm D or Social Worker employed by Erlanger Medical Center is responsible for direct and/or indirect care of patients under the supervision of a Physician or Nurse Practitioner/Physician Assistant.
Position is involved in a team‑based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. The Diabetes Educator is trained and assigned to coordinate care for patients, support patients/families/caregivers in self‑management, self‑efficacy and behavior change, and manage the practice’s patient population. The role participates in the practice’s quality improvement process and performance evaluation, and teaches diabetes self‑management in both individual and group settings.
Education- Graduate from an approved program in dietetics/nutrition or be a registered nurse or certified diabetes educator.
- Pharm D or Social Worker eligible for a Certified Diabetes Educator designation.
- A minimum of three years of progressive experience in a hospital or clinic setting.
- Experience in insulin pump education.
- Experience in teaching diabetes education (preferred).
Current professional registration required. BLS certification prior to and maintained during employment.
Department Position SummaryThe Diabetes Educator establishes and coordinates diabetes self‑management training for adult patients. Responsibilities include client‑centered case management, coordinating multidisciplinary plans of care, assessing patient data, providing education to adult learners, teaching diabetes self‑management to individuals and classes, and assisting with activities that promote and encourage diabetes education as assigned.
