• Partner with members and families to identify supports and services for achieving personal health and independent living goals
• Collaborate with a team of clinical and social support colleagues to meet each member's physical and behavioral health needs
• Conduct thorough face-to-face assessments to determine members' current status and needs
• Authorize and confirm the provision of appropriate home and community-based services
• Maintain compliance with licensing and immunization requirements

• Minimum of 3 years of experience in a clinical setting
• Active Registered Nurse license in Tennessee or a participating Nurse Licensure Compact Act state, or a Master's in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW)
• Exceptional customer service skills
• Must reside within Stewart and Montgomery Counties
• Valid Driver's License

• 2 years of clinical experience
• Proficiency in basic Microsoft Office applications and email
• Effective time management skills
• Strong oral and written communication abilities
• Excellent interpersonal and organizational skills
