• Under the supervision of a Registered Nurse, provide skilled nursing services as ordered on the plan of care and according to policy/procedures and state practice acts.
• Instruct the patient/caregiver in various aspects of patient care and disease management. Document the patient/caregiver comprehension of and compliance with teaching.
• Observe patient response to care/teaching and make recommendations regarding revision of the plan for patient care to the Registered Nurse.
• Provide and document skilled nursing care according to the established plan of care, policies/procedures, and standards of care. Perform other duties as assigned by the RN / Clinical Manager.
• Assist with home health care tasks as needed.

• Unencumbered, current Licensed Practical Nurse license in the state where the Home Care agency is located or compact state if applicable.
• Graduate of a state approved LPN program.
• Minimum one (1) year clinical experience as an LPN. Individuals with less than one full year’s experience may be considered and will receive mentorship.
• Experience in home health care is desirable.
