Position
Travel Licensed Practical Nurse (LPN) PRN – Hospice Travel LPN.
Pay$20 / hr – $35 / hr depending on experience and responsibilities.
Responsibilities- Provide quality patient care and perform technical skilled care in the patient’s home.
- Assist home health aides in fulfilling their responsibilities.
- Travel within Central Tennessee as a Regional Travel Nurse.
- Provide technical care to patients, receive assignments and reports, provide prescribed medication under supervision of Registered Nurse, in accordance with agency policies and as permitted by local and/or state regulations.
- Observe, record, and report all observations of the patient’s mental and physical care provided in progress notes and submit notes daily to the Patient Care Manager.
- Obtain complete report on patients to be seen prior to visits and give complete report of care done during visits to Patient Care Manager.
- Perform special procedures/treatments supervised by an RN (e.g., IVs, NG tubes, draw blood).
- Notify the Patient Care Manager and/or physician immediately of any changes in patient’s condition that warrant attention; document changes in the clinical record.
- Participate as an IDT team member.
- When requested, assist home health aides in performance of their duties, assist the physician or the RN in performing specialized procedures.
- Participate in on call according to rotating schedule.
- Willingness to travel to other offices.
- Must be licensed in the agency state of operation.
- Must have a complete knowledge of technical skills required in the performance of nursing procedures with at least one (1) year of nursing experience.
- Must successfully complete on‑site evaluations and competencies within ninety (90) days of employment and bi‑annually thereafter.
- Possess an understanding of the concepts of hospice care and role as a member of an interdisciplinary care team.
