Job Summary

The Stroke Coordinator assures that patients presenting to Erlanger with risks or symptoms of stroke receive optimal care using established standards and organizational goals. The Coordinator maintains high personal and staff effectiveness in prompt recognition and care of stroke or non-stroke emergencies, including blood pressure management, administration of thrombolytic agents, and research. The Coordinator functions as regional and intra‑hospital Coordinator for the Stroke Program, planning, organizing, and implementing continuity of stroke care, continuous process improvement, patient, public, and professional education, protocol development, weekly interdisciplinary stroke rounds, and positive relations among affiliated network hospitals, medical centers, physicians, and external agencies to promote program design and promotion.

The Coordinator is also responsible for the organization and implementation of Telemedicine, providing leadership and coordination of program activities, overseeing programming, community outreach, daily operations, and integration of telemedicine with clinical programs. The Coordinator demonstrates an ability to organize, prioritize, meet deadlines, and work with teams across departments and regional hospitals, ensuring effective communication to maximize patient care through a regional telehealth approach.

Education

    Graduate from an accredited School of Nursing, BSN

Position Requirements
  • Current license to practice nursing in the state of Tennessee. RN
  • Bachelor of Science from an accredited nursing school. Certification in specialty (Preferred)
Responsibilities
  • Work with Service Line Administrator on preparation of TJC/DNV certification for comprehensive stroke‑centered designation.
  • Maintain cooperative relationships with individuals representing other network hospitals and medical centers to coordinate educational programs, patient transfers, referrals, and quality care services.
  • Collect, analyze, trend, and report required statistics to maintain stroke network designation status at Erlanger Health System.
  • Main contact person for all Stroke Network hospitals and Erlanger Health System for clinical expertise, education, and protocol updates.
  • Act as contact person for family members during hospitalization.
  • Communicate with regional and local primary care physicians about patient progress and status; route complaints and issues through appropriate processes.
  • Develop and implement cerebral vascular policies and procedures, including coiling care maps, TPA administration, preprinted orders, patient education packets, and critical pathways.
  • Evaluate team compliance with department standards and assess learning needs, providing opportunities and attending educational meetings to enhance nursing skills and competencies.
  • Provide support and clinical expertise to care teams for complex stroke cases that do not achieve expected outcomes.
  • Monitor trends and ensure patient outcomes meet established quality levels; collaborate with professionals to plan LOS, discharge, and follow‑up.
  • Lead community and professional education, stroke risk screenings, health fairs, stroke awareness activities, and monthly community engagement.
  • Maintain positive relationships with medical personnel, resolving operational problems and maximizing use of comprehensive stroke and health system resources.
  • Support and participate in continuous quality improvement efforts to increase patient outcomes, satisfaction, and efficient use of resources.
  • Apply analytical abilities to identify program needs requiring intervention or revision.
  • Serve as liaison with administration, network engineers, support personnel, and vendors to address clinical and educational telehealth program needs.
  • Maintain awareness of telemedicine program status and potential issues.
