NHS is seeking a Practice Nurse to join a small team of two other nurses and two HCAs. The post holder will deliver nursing services to the patient population within a primary care setting and support the practice multidisciplinary team in policy and procedure delivery.
The role covers chronic disease management, health promotion and routine nursing duties, with responsibilities to support safeguarding and vaccination programmes. Part time hours will be discussed.
