• Provide high-quality nursing care to patients in their homes
• Develop and implement individualized patient care plans
• Administer medications and treatments as prescribed
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes in condition in accordance with agency-defined time frames

• Valid Registered Nurse license in Massachusetts
• 1‑2 years of experience as a Registered Nurse
• 1 year of experience in home health care required
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Reliable transportation and willingness to travel within the service area
