Overview

Gloucestershire Health and Care NHS Foundation Trust is seeking a part-time Mental Health Community Support Worker on a permanent basis. The role is 22.5 hours per week. Closing date: 18 February 2026. The post is based in Cheltenham within the Recovery Team and focuses on supporting adults living with severe mental illness to regain stability, independence, and an improved quality of life.

You will work as part of a multidisciplinary team, collaborating with service users, internal colleagues (nurses, occupational therapists, psychologists, peer workers) and external health and social care partners to deliver a coordinated, person-centred approach to recovery.

Main duties and responsibilities
  • Care coordination: Provide care coordination for an allocated caseload of service users, ensuring needs are assessed, reviewed, and met through personalised care plans.
  • Goal-focused interventions: Deliver recovery-oriented interventions to help individuals develop skills, confidence, and independence.
  • Group work and clinics: Facilitate and contribute to group work and therapeutic clinics, supporting service users to engage in structured activities that promote wellbeing.
  • Collaborative working: Work with internal team members and external partners to ensure high-quality, person-centred care.
  • Documentation: Maintain accurate, timely documentation in line with professional and organisational standards.
  • Risk and care planning: Participate in multidisciplinary discussions and contribute to risk assessments and holistic treatment approaches.
  • Knowledge sharing: Share knowledge, experience, and skills with colleagues, students, and new staff to support learning and continuous improvement.
  • User involvement: Promote service user involvement and uphold values of dignity, respect, and empowerment in all aspects of care.

Qualifications and experience
  • Qualifications: NVQ level 3 or equivalent competency-based training in a relevant health or social care area, plus theoretical and practical knowledge to diploma level equivalent, or equivalent through approved associate practitioner training, relevant foundation degree, or commitment to obtain within 2 years. IT skills.
  • Experience: Experience in mental health or social care services at Bands 2, 3 or 4, ideally in a community setting. Knowledge of CPA process, care planning, risk assessment, and recovery-based care. Clinical skills training (awareness of physical health and wellbeing). Knowledge of the Voluntary Care Sector and partnership working.

Job responsibilities

The Band 4 Community Support Worker plays a vital role within the Cheltenham Recovery Team, supporting adults living with severe and enduring mental health conditions to achieve meaningful recovery goals and improve quality of life. You will deliver person-centred, community-based interventions and ensure service users are actively involved in their own care and treatment. This part-time role offers a significant opportunity to develop professionally while coordinating care, promoting independence, and encouraging engagement with therapeutic and practical support services.

Ongoing development, supervision, and training are integral to the role.
