Fixed term contract up to 12 months, part time 30 hours a week.

Supports the Public Health Nursing Team within the 0-5 Health Visiting Service, focusing on families with children with additional needs.
Main duties- Work with children and families in the community, ensuring effective, safe, inter‑professional collaboration.
- Support families and children with SEND and promote progression through GHC policies such as the SEND Graduated Pathway, SCAAS, safeguarding and health promotion.
- Provide care under the instruction and supervision of a Senior Clinical Public Health Nurse – Health Visitor following GHC protocols.
- Assess and implement Healthy Child Programme interventions and other evidence‑based programmes in partnership with clients and agencies.
- Progress interventions based on identified risks to children’s health outcomes.
- Lead by example, represent nursing values and align to GHC Values Framework.
- Assess global health needs of communities and children to reduce disadvantages and promote wellbeing.
- Monitor and promote the health of all children, identifying physical, emotional, mental health and development needs.
We provide a multi‑professional preceptorship programme aligned to the National Preceptorship Framework. All newly qualified nurses, nursing associates, internationally educated practitioners, allied health professionals and return‑to‑practice practitioners can enroll. An additional Level 7 (Masters) 15‑credit module (AP7026) is available.
Person specification- Registered Nurse / Midwife
- UK driving licence
- Registered with the Nursing and Midwifery Council (NMC)
- Educated to degree or diploma level
- Sound understanding of Public Health Nursing, Healthy Child Programme, NHS Plan and national policies
- Knowledge of evidence‑based practice in health visiting role
- Accurate clinical record keeping following national and local guidelines
- Excellent interpersonal and communication skills, English fluency
- Ability to organise and prioritise workload
- Ability to build working relationships with colleagues, clients and stakeholders
- Good listening, communication, presentation and networking skills
- Ability to undertake statutory and mandatory training
- Experience working with children who may have additional needs
- Ability to cope with a frequently changing professional climate
- Experience identifying and assessing health and community health needs
- Understanding and applying research in practice
- Experience working across agencies

Annual salary range: £32,073 to £39,043 per annum (pro rata).
