Role Overview
Community nursing role delivering nursing care in patients’ homes, providing information and education to enable patients to enjoy maximum independence, health and well‑being.
Key Responsibilities- Assess, plan, implement and evaluate patient home care plans.
- Provide direct nursing care and support to patients and carers.
- Complete patient records and paperwork in real time using laptop and smartphone.
- Work within the 8am to 8pm shift system, covering early, late, weekend and bank holiday shifts.
- Collaborate with multidisciplinary team via mobile technology.
- Current NMC registration.
- Post‑registration nursing experience (newly qualified nurses not accepted).
- Experience in community or home‑based nursing preferred.
- Excellent communication skills and clinical decision‑making ability.
Permanent position, full‑time (37.5 hours per week) with a flexible working policy covering all seven days, including weekends and bank holidays.
