Radis Community Care in Chepstow is seeking an Evening Field Care Supervisor (driving essential) to join our community team.
You will conduct reviews, spot checks and carer assessments, complete risk assessments and MAR charts, and develop support plans in collaboration with NHS, Social Services and GPs.
This role requires a driving licence and access to a vehicle; all supervisor hours are paid at an enhanced rate with travel time and mileage compensation.
