Hospice Registered Nurse Case Manager (RN)
Join our team as a Hospice Registered Nurse Case Manager (RN). This role supports the hospice care team by managing patient assessments, care plans, and patient and family education.
Responsibilities- Completing an initial assessment of patient and family to determine hospice care needs
- Regularly re-evaluating patients’ nursing needs
- Using assessment data to determine care plan needs
- Collaborating with the entire healthcare team, patient, and family to direct the work of the team to provide care that is continuous and well-coordinated
- Counseling the patient and family in meeting end‑of‑life needs
- Initiating appropriate preventative and palliative nursing procedures
- Communicating with the physician regarding patient’s needs and reporting any changes in the patient’s condition
- Supervising the Hospice Aides

- Associate's Degree in Nursing
- Must have a State Certified RN license
- 2 years’ experience as a professional nurse, hospice experience preferred
- Must have a valid Driver’s license and reliable transportation
- Must be willing to travel
- Strong verbal and written communication skills.
