Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join either one of their Neighbourhood, Transitions or LD Team. Fantastic payrate of £30 per hour! Learning Disabilities (LD) Team Undertake assessments, reviews, and support planning for adults with learning disabilities, promoting independence and person-centred outcomes. Manage safeguarding, risk, and Mental Capacity Act responsibilities in line with legislation and best practice. Work collaboratively with families, health professionals, and partner agencies to coordinate effective support and achieve positive outcomes Transitions Team Support young people transitioning from Children's Services to Adult Social Care through assessment, planning, and review processes.
Promote independence and positive outcomes by working closely with young people, families, education, health, and partner agencies. Manage safeguarding, risk assessment, and statutory responsibilities in line with relevant legislation and guidance. Neighbourhood Team Complete Care Act assessments, reviews, and support planning for adults with a range of care and support needs. Manage safeguarding enquiries, risk assessments, and Mental Capacity Act duties to ensure individuals' safety and wellbeing.
Work in partnership with residents, carers, and multidisciplinary teams to deliver strengths-based support and positive outcomes. Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England. Must have minimum of 3 years post qualifying social work experience
