Overview

Summary:

In accordance with the National Scope and Standards of Diabetes Educators, as well as the National Standards for Diabetes Education and Support, the Dietitian assesses the needs of patients and designs, implements and evaluates a teaching plan. The dietitian provides individual nutritional counseling in the following areas: diabetes, weight management, hyperlipidemia, cardiac risk management, hypoglycemia, general nutrition wellness, pregnancy, gestational diabetes, GI-related disease problems, and pulmonary nutrition.

The Dietitian is also responsible for facilitating and assisting in the development of programs for group sessions, seminars, cooking demonstrations, supermarket tours, and lectures for the local businesses/community. Interacts with an interdisciplinary team of health professionals. Promotes positive behavior change with client population using motivational interviewing techniques, goal setting and effective client communication and support. Evaluates and assists clients with barriers to change.

Essential Duties and Responsibilities

These duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.

  • Provide one-on-one and group outpatient medical nutritional counseling
  • Conduct group sessions relative to weight loss and maintenance, diabetes management, cardiac risk management, pulmonary management, and general nutritional wellness
  • Provide diabetes self-management education and support in all 9 content areas as defined by the National Standards of Diabetes Self-Management Education and Support
  • Assess the nutritional needs of outpatients and develop nutritional care plans with prescribed therapeutic diets for nutritional problems
  • Educate patients about behavior modification relative to diet
  • Document all information on the progress of each individual in patient charts and in the electronic health record
  • Communicate with patient's provider or provider's office to report on patient's progress
  • Assist in the implementation of the assessment plan, and in the implementation and evaluation stages of the scientific process
  • Assist in developing and revising programs for individual and group interventions
  • Participate in the Diabetes Program Quality Improvement Plan
  • Participate on the Diabetes Advisory Committee
  • Facilitate programs, seminars, cooking demonstrations, supermarket tours, and lectures for the local businesses, the community, and the Lifestyle Center
  • Meet with local employers to determine employee needs and nutritional services being offered
  • Assist in answering phones to provide potential clients with program information
  • Provide information to patient program participants and the general public regarding Lifestyle Center programs and related services
  • Make appropriate referrals to the Lifestyle Center programs
Qualifications

Education and Experience: Minimum Required

Education:

Successful completion of a Registered Dietitian program.

Preferred Education:

Bachelor or Master of Science Degree in Nutrition, Dietetics.

Experience:

2+ years in teaching nutritional concepts in a hospital or outpatient nutritional program.

Certifications: Applicants must be a Registered Dietitian through the Commission on Dietetic Registration (CDR) with current CDR status.
