Full Time Home Care Service Coordinator – Addus Home Care
Position Summary: Coordinate the scheduling and supervision of in‑home care workers and clients in the Chesapeake, VA area. Supervise direct service employees, manage client care plans, and ensure compliance with Medicare, Medicaid, HIPAA, and agency policies.
Responsibilities- Create and manage work schedules, handling changes due to client requests, illness, vacation, or leaves of absence.
- Provide alternate coverage to maintain uninterrupted client care plans.
- Contact care providers and clients to update services and conduct monthly client wellness calls.
- Perform home visits as required and resolve escalated client complaints and theft claims.
- Supervise direct service employees, setting expectations for attendance, performance, and conduct.
- Maintain a professional branch environment that is neat, clean, and orderly.
- Uphold confidential handling of sensitive information and adhere to all regulatory requirements.
- Demonstrate reliable transportation and consistent attendance.
- High school diploma or equivalent.
- At least 1 year of experience in the home health or related industry.
- Strong interpersonal, organizational, and communication skills.
- Proficient computer skills, including Microsoft Word, Excel, and scheduling programs.
