Physical Therapist – Therapeutic Consultation (VA DD Waiver)
Community Health and Wellness (CHAW) seeks a Physical Therapist (PT) to provide therapeutic consultation under Virginia’s Developmental Disabilities Waivers (FIS/CL). The role is consultative: assess needs, create a written PT support plan, train and coach families, caregivers, and DSPs, and monitor outcomes. This position does not provide ongoing direct therapy.
Responsibilities- Conduct PT assessments and observations.
- Develop written PT support plans with strategies and adaptations.
- Demonstrate interventions and train families, caregivers, and DSPs.
- Participate in annual ISP meetings and team reviews.
- Complete Medicaid‑compliant documentation (notes, reviews, training logs, summaries).
- Physical Therapist licensed in Virginia.
- Experience working with individuals with /DD.
Unit = 1 hour; in‑person or HIPAA‑compliant teleconsultation. Frequency determined by assessed need and ISP authorization.
