At Matrix Medical Network, we’re looking for passionate Nurse Practitioners or Physician Assistants to conduct comprehensive in-home health assessments. This full-time role allows you to focus on medical history, diagnosis, treatment, and education while collaborating with a dedicated team of healthcare professionals.

As a valued member of our team, you will:

• Conduct thorough adult and geriatric assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with physicians, case managers, social workers, and caregivers to ensure comprehensive patient care.
• Provide services in the comfort of patients' homes or skilled nursing facilities.

To thrive in this role, you should possess:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP/PA licensure in your state, including prescription authority.
• Board certification in relevant specialties (NCCPA, AANP, AACN, or ANCC).
• Current BLS, ACLS, or CPR certification.
• A year of NP or PA experience is preferred, but new graduates are encouraged to apply!
• A valid state driver's license and proof of adequate automobile insurance.
• Strong communication skills and proficiency in medical record software.

Our full-time providers typically work Monday to Friday, from 8:00 AM to 5:30 PM, with assessments conducted from Sunday to Saturday between 8 AM and 7 PM. You may need to travel up to 15% of the time, including overnight stays.
