Overview
Nurse Practitioner - Primary Care physician employment in Virginia
Role DetailsMonday-Friday role. Schedule revolves around patients’ dialysis unit visits, primarily between 8:00 AM and 5:00 PM. Work from a portfolio of 3-8 dialysis centers. Clinical autonomy to determine which patients to see and during which time.
Average 10-12 patient visits per day.
Responsibilities- Coordinate patient follow-ups to proactively address concerns, including coordinating care with other providers, ordering key labs, and prescribing medication.
- Build clinical relationships with nephrologists, dialysis teammates, integrated care teammates, and local PCPs and specialists.
- Conduct annual comprehensive health evaluations.
- Minimum of 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
- NP license to practice autonomously.
- VA DEA and comfort in prescribing for chronic diseases typically associated with ESRD, such as congestive heart failure.
Experience with dialysis or nephrology.
