Reports To: Nurse Manager / Nurse Practitioner
The Licensed Practical Nurse (LPN) provides direct nursing care and medication support to adults receiving short-term residential crisis stabilization services. The LPN works under the supervision of a Registered Nurse and/or Psychiatric Nurse Practitioner to support individuals experiencing acute mental health crises, ensuring safe medication administration, health monitoring, and coordination with the interdisciplinary treatment team.
This position is essential to maintaining medical safety, continuity of care, and regulatory compliance within the Residential Crisis Stabilization Unit (RCSU).
Essential Duties and Responsibilities Nursing & Medical Support- Administer prescribed medications in accordance with physician/psychiatric provider orders and agency medication management policies.
- Monitor residents for medication effectiveness, side effects, adverse reactions, and changes in physical or mental status.
- Complete medication counts, storage checks, and documentation per DBHDS and DMAS standards.
- Perform basic nursing assessments and health screenings as assigned.
- Monitor vital signs and respond appropriately to medical concerns.
- Support residents during periods of acute emotional or behavioral distress using trauma-informed and recovery-oriented approaches.
- Collaborate with clinical staff during psychiatric or medical crises.
- Assist with crisis interventions and follow approved emergency protocols.
- Communicate relevant medical and behavioral observations to the RN, Nurse Practitioner, and interdisciplinary team.
- Participate in treatment team meetings, shift reports, and care planning as required.
- Assist with discharge planning and coordination of follow-up medical and psychiatric services.
- Maintain accurate, timely nursing documentation, including MARs, progress notes, incident reports, and health records.
- Ensure compliance with HIPAA, DBHDS regulations, DMAS requirements, and agency policies.
- Follow infection control, safety, and emergency procedures at all times.
- Promote a safe, structured, and therapeutic residential environment.
- Ensure medical supplies and equipment are properly maintained and available.
- Support staff training related to medical safety and health monitoring as appropriate.
Required Qualifications
- Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Virginia.
- Graduation from an accredited practical nursing program.
- Experience in behavioral health, psychiatric, crisis stabilization, or residential settings preferred.
- Strong knowledge of medication administration and documentation standards.
- Ability to remain calm, professional, and effective in high-acuity crisis environments.
- Experience working with individuals experiencing severe mental illness and/or co-occurring substance use disorders.
- Experience in crisis services, inpatient psychiatric units, or residential treatment programs.
- Familiarity with DBHDS and DMAS regulations.
- CPR/First Aid certification
- CPI / Crisis Prevention or equivalent de-escalation training
- TB screening, background check, and all pre-employment clearances per DBHDS requirements
- Ability to stand, walk, bend, and lift as required during shifts.
- Ability to respond quickly in medical or behavioral emergencies.
- Work is performed in a 24/7 residential behavioral health environment with individuals in acute crisis.
