About the role

The Associate Advanced Nurse Practitioner – Community provides specialist palliative and end‑of‑life care to patients in their own homes or preferred place of care. The role raises clinical standards, promotes a learning environment for other healthcare providers, and works autonomously as part of the Community Palliative Care Team under Locality Lead supervision.

Key Responsibilities
  • Provide support to patients with palliative or end‑of‑life needs at home, raising clinical standards and offering a learning environment for colleagues.
  • Plan, assess, implement and evaluate care in partnership with multidisciplinary teams and out‑of‑hours providers.
  • Create an environment that encourages patients and families to actively participate in care decisions and voice their wishes.
  • Deliver advanced clinical decision‑making through assessment and interpretation of clinical information for patients, families and carers with complex or rapidly changing symptoms.
  • Assess, plan, implement and evaluate patient care needs, including hospital admission and advanced care planning to identify preferred place of death.
  • Provide appropriate pain and symptom management, and direction for the psychological and spiritual needs of patients, families and carers.
  • Support, inform and advise patients, families, colleagues and other health professionals, including the Mountbatten Care Co‑ordination Centre.
  • Prioritise daily duties in response to patient and family requirements while maintaining quality and compliance with the NMC Code of Practice and Mountbatten values.
  • Be fully conversant with organisational medicines policies, palliative care guidelines and administer medication in line with control procedures.
  • Provide training support on syringe drivers and teach staff nurses and students in collaboration with the Community Team and other professionals.
  • Promote infection prevention and control policies and procedures in line with Mountbatten Infection Control policy.
  • Advocate for patients and families, ensuring their views are represented, especially during advance care planning.
  • Communicate complex and sensitive information effectively in written and oral form to patients, families, carers and other professionals.
  • Organise tasks, prioritise and make effective decisions with strong organisational skills.
  • Maintain physical capability for manual handling responsibilities required by the role.
Qualifications and Skills
  • First Level Registration with the NMC.
  • Relevant degree in Health/Nursing Studies or equivalent.
  • Full UK driving licence (clean) and availability to use a vehicle with business insurance.
  • Minimum of 2 years post‑registration clinical practice experience caring for vulnerable adults in a care, hospital or hospice setting.
  • Experience collaborating within multidisciplinary teams and building good working relationships at all levels, including volunteers.
  • Demonstrable experience providing care to vulnerable adults in a hospice, care or hospital environment.
Working Conditions

Part‑time: 30 hours per week, worked flexibly to meet a shift rota that includes days, nights, weekends and bank holidays. A clean driving licence and use of a vehicle with business insurance are essential.
