CDDFT Community Respiratory Nursing Service is pleased to advertise this exciting opportunity for a permanent, 24 hours per week, over 3 days, Community Respiratory Specialist Nurse, Band 6, working across the North of the service including North Durham, Chester‑Le‑Street and Derwentside and Easington localities. The successful postholder will deliver respiratory care to patients including COPD, Emphysema, Asthma, Bronchiectasis, and Interstitial Lung Disease, in out‑patient clinics as well as in their own homes, supporting the community specialty respiratory nursing service.
The successful candidate will utilise their outstanding organisation and communication skills to achieve best patient outcomes whilst being well supported by expert respiratory specialists to develop a range of competencies in delivering excellent respiratory care and support for this group of complex patients.
Main duties of the job- Undertake specialist, holistic assessment, diagnosis, planning, implementation and evaluation of clinical care for a caseload of patients with complex respiratory disease.
- Be knowledgeable in respiratory disease, pathophysiology, function and management.
- Provide specialist advice to patients/carers.
- Be skilled in oxygen assessment, review and prescribing.
- Act as an accountable practitioner.
- Maintain accurate records as an autonomous practitioner.
- Act as point of contact for the wider local community teams.
- Provide education, mentorship and leadership for junior colleagues and students.
- Skilled at assessing and managing risk.
- Accept responsibility for decisions made and actions taken.
- No requirement to manage/monitor budget.
We provide hospital services from two acute sites – Darlington Memorial Hospital and University Hospital of North Durham – and a centre for planned care in Bishop Auckland with care from community hospitals in Chester‑Le‑Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community‑based settings and in patients’ homes. - Identify need for change and support management of change, including service development.
- Develop negotiation and influencing skills.
- Provide direct management for band 5, 4 and 3 colleagues.
- Coordinate clinical work with colleagues demonstrating effective time‑management skills and participate in the monitoring and reviewing of team performance.
- Commit to continuous personal development by attending mandatory training, using eLearning and adhering to all trust policies and procedures.
- Understand own role within the wider multidisciplinary team and articulate own development needs through attending clinical supervision, 1:1 sessions, undertaking appraisal preparation and contributing to objective setting and personal development planning.
- Possess effective leadership skills by directly managing junior staff within the teams.
- Perform risk assessments and compile action plans for the service.
- Be an active part of the recruitment process, including short listing and interviewing.
- Participate in and provide clinical supervision.
- Promote service development, improvements and quality expansion and efficiencies.
- Case‑manage patients with a complex respiratory diagnosis within a community setting without direct supervision, reporting to the respiratory specialist nurse lead as appropriate and utilising specialist history‑taking and examination skills.
- Holistically assess patient needs and develop an individualised plan of care that meets the needs of patients and carers through physical assessment skills and a respiratory accredited course.
