Palliative Care Nurse – Community

County Durham and Darlington NHS Foundation Trust is one of the largest integrated care providers in England, serving around 600,000 people. This Band 6 post offers an exciting opportunity for a suitably qualified and experienced nurse to work within the Specialist Palliative Care Team in the Community North area.

Closing date: 16 July 2026.

The Trust provides services from two acute sites – Darlington Memorial Hospital and University Hospital of North Durham – and a network of community hospitals and care settings.

Core hours

09:00 – 17:00, Monday through Friday. Weekends and bank holidays are rostered across the team as per CDDFT policy.

Responsibilities
  • Provide consistent, high‑quality specialist palliative care service for patients and families.
  • Assess, plan, implement and evaluate care for patients and carers.
  • Supervise and provide clinical supervision to others.
  • Respond to difficult situations with sensitivity and compassion.
  • Develop and maintain effective working relationships within the team.
  • Contribute to the development of specialist programmes of care and support R&D activities.
Special Requirements
  • Ability to travel across Trust sites.
  • Flexible working hours when required.
  • Valid full UK driving licence (for driving posts).
Experience and Professional Knowledge
  • Experience working as a Nurse.
  • Proactive approach to palliative and end‑of‑life care.
  • Knowledge of policies, guidelines and professional standards.
Special Skills & Knowledge
  • Communication of complex, sensitive, confidential information with empathy and reassurance.
  • Assessment and interpretation of complex needs of patients/carers.
  • Planning of care provision, including clinics and health education.
  • Clinical care delivery (injection, catheter insertion, infusion, etc.).
  • Providing specialist advice and impartial advice to patients/carers.
  • Implementation of policies and proposal of changes to working practices.
  • Clinical supervision and training of staff and students.
  • Lead at specialist, area or team level; may manage a caseload in the community.
Statutory Registration
  • Registered Nurse (adult) on part 1 of the NMC register.
  • Valid UK NMC PIN.

Salary
