We are seeking to appoint an experienced senior palliative clinical nurse specialist to work within the Rural Alliance Care Community Team as part of Specialist Palliative Care service within Cheshire Wirral Partnership. You will also work as part of the wider palliative care team across the footprint, which includes palliative care consultants, clinical nurse specialists, occupational therapy, physiotherapy, therapy assistants and the local hospice.
Experience of working in the community is desirable as the role is based supporting patients in their own homes.
Candidate ProfileWe are looking for an experienced and well‑motivated Clinical Nurse Specialist with excellent communication, organisation and leadership skills. The successful candidate will receive full supportive induction and will work closely with the wider multidisciplinary team.
Responsibilities- Provide holistic care and support to patients with complex palliative care needs with malignant or non‑malignant life‑limiting conditions, working autonomously and as part of a team.
- Deliver care to patients registered with a West Cheshire GP in their own home.
- Advise, educate and support patients, their carers and other health care professionals.
- Promote advanced care planning, manage complex end‑of‑life symptoms and patient and carer expectations while meeting the holistic care needs of the patient.
